Experience with the Harrington erisophake in fifty cataract extractions.

چکیده

IN 1951, Dr. David Harrington of the Division of Ophthalmology, University of California, San Francisco, sent me one of his erisophakes. Since then I have used the instrument sporadically, by which I mean that in an operating list oftwo, three, or four cataract extractions, an occasional one was done with the erisophake, the rest in the ordinary way. Fifty such extractions have now been performed and a note of my experience may be of some interest to others who may contemplate using this instrument. The phakoerisis of Barraquer Barraquer and Barraquer Moner in Barcelona is very impressive to watch, and the results obtained at their hands seem excellent, but the space-and-time-machine-like meter on one of the operating theatre walls which has to be watched and consulted, and on which the amount of suction seems to depend, is rather awe-inspiring.

The Esposito Erisophake and Cataract Extraction.

IN 1953 I described an erisophake (Esposito, 1953), which I had devised in 1950, and which I had been, and have been, using since that time in cataract surgery. Since the original article appeared, this instrument has been used successfully and extensively by many ophthalmologists as noted by many writers, including Hope-Robertson (1954) and Givnor (1953). Of interest has been the attempted mod...

Cataract Extraction by Expression.

BEFORE the introduction of enzymatic zonulysis by Joaquin Barraquer (1958), the normal procedure of senile lens extraction was either by intraor extra-capsular extraction, or, more recently, by the use of the erisophake. This latter operation can be done with much greater facility by the use of the enzyme, as has been demonstrated by Barraquer (1958), Cogan (1958), and Cogan, Symons, and Gibbs ...

Intracapsular Extraction of Intralenticular Magnetic Foreign Bodies.

INTRALENTICULAR magnetic foreign bodies if left untreated lead in most instances to total cataract formation, siderosis, or as a later complication, loss of the eye. Since the use of the magnet has been developed, and powerful hand instruments have made manipulation easier, intralenticular magnetic foreign bodies have been removed by this means via the anterior route. The risk of siderosis is t...

Some observations on the use of chymotrypsin.

I AM unfortunately not one of those who can say "I always do an intracapsular extraction", or even that I always try an intracapsular extraction, because I think some cases are safer and better with an extracapsular extraction of cataract. After the history has been noted, each case is carefully examined with the slit lamp and a decision is taken which operation will be best for that particular...

Scleral Ledge in Cataract Incision: a Cause of Rupture of the Capsule.

Most surgeons seem to favour the former method. Even when the erisophake is used instead of forceps, the technique of zonulolotomy and delivery of the lens remains essentially the same. Since the introduction of A.C.T., however, mechanical zonulolotomy has been replaced by enzymatic zonulolysis and the extraction of the lens by seizing it at the 12 o'clock position is being increasingly practis...
